A neat trick I was told was "always have the keys in your hand before you shut the door". It seems to work for me.
Mind you, I forgot once to lock James's car because Charles was throwing a tantrum when we were getting out, and it got robbed :( Now I am slightly obsessive about checking I have locked a car, and have been known to walk back halfway across a carpark if I'm not sure.
Also I have two keyholders to my house, in case of emergency or stupidity.
