I'm still regularly tweaking my todo list system, trying to find what works for me. And trying to find something that isn't regularly purged as I lose track and start over with the things I *really* need to do this week.

I think I need to move to a database based system, where I reliably orthogonally categorise things. (Eg. things I must do this week, and things I want to do, both of which may have some items put in "do today" :))

The current tweak is to have a "worried" section. My greatest failure mode is when I leave something undone and it becomes a massive bugbear and I'm scared to anything. Anything that might be getting that way -- or anything I just want to keep in mind, but not do anything specific about -- goes there. And then if I start to lose it I can drop everything else and face up to that until I'm in sync with reality again :)

I've also started transferring regular chores to a web-based reminder service, and thence to my inbox, which is a natural part of the day's todos, and I can either do them, or, if necessary, delete them in the confidence they'll come back next week and not be forgotten forever.
